AGREEMENT |

AND RELEASE*************************
I,
__Attendee/Registrant_____,
of sound body and mind, in consideration
of my participation at The Ulitimate
Urban Weekend Getaway hosted by BlackNLA.com,
over the dates of Friday, June 18, 2010
to June 20, 2010, agree to the following:
1. BLACKNLA utilizes transportation
provided by carriers who are not employees
of BLACKNLA and I will not hold BLACKNLA
responsible for the default of any such
carrier or its personnel. I also agree
that I will pay any additional expense
incurred due to any delays in service,
weather, strike, or any other cause
which are not the direct fault of BLACKNLA.
2. I understand that BLACKNLA will immediately,
upon being informed of any illness or
injury to myself, will make a reasonable
effort to advise my emergency contact
of the nature and extent of such illness
or injury. In the event that I become
ill or injured and someone cannot be
reached, I hereby authorize a representative
of BLACKNLA to proceed with all medical
treatment recommended by a physician
at my expense.
3. I agree that in the event that it
is necessary in the opinion of the BLACKNLA
to transport me home due to illness,
injury or misbehavior, I agree to pay
for the cost of transportation.
4.
BLACKNLA reserves the right to change
or alter activities listed in package.
Equivalent arrangements will be made
for any substituted activity. All changes
will be displayed on website and sent
in final reservation notices.
5. If I or any of my possessions are
injured, damaged, or lost due to an
accident which occurs while I am in
a vehicle or airplane hired by BLACKNLA
during the period of this trip, I agree
that BLACKNLA is not responsible for
the loss, injury or damage to any of
my possessions whether said injury,
loss, or damage occur as a result of
accident or for any other reason while
I am on the BLACKNLA weekend getaway.
6. In the event that I behave in an
unbecoming manner in the judgment of
a representative of BLACKNLA, then BLACKNLA
may send me home at my expense in that
regard. I agree that BLACKNLA reserves
the right to change schedules or cancel
any part of this getaway weekend.
7. If for any reason or circumstance
beyond the control of BLACKNLA it becomes
necessary for BlackNLA to cancel the
trip prior to its commencement or during
the trip then only unused fees will
be refunded. Financial commitments and
payments are made weeks and months prior
to a trip: therefore, certain fees are
used and not refundable to BLACKNLA
- these fees are in the form of transportation,
lodging, venue, FNB and office services.
Refunds are made within 60 days after
notice of cancellation
8. I acknowledge that boating, jet skiing
and other water activites are dangerous
activities which can result in serious
physical injury or even death to any
person participating in it. I hereby
release BLACKNLA, its employees and
representatives' any and all liability
which may arise as a result of any injury
or death, which occur during this vacation
while participating any boating, jet
skiing, water sports or other activites
during the weekend getaway.
9.
BLACKNLA is not responsible for any supervision
or problems resulting from air or ground
travel, including ground transportation
to and from airports and venues, lodging
facilities, and residence locations.
10.
BLACKNLA will be using photographers and
videographers on the weekend event to
document participant activities. Photos
and video will be posted on BlackNLA or
one of its affiliate sites.

BNLA,
Inc dba BlackNLA, is acting as intermediaries
for suppliers and operators, such as hotels,
tour operators, travel agent, charter boat
services, ground transportation companies,
and independent firms. BlackNLA does not
own or operate any of these firms; therefore,
shall not be responsible for breach of contract
of any intentional, unintentional, or careless
actions or omissions on the part of such
suppliers which result in any loss, damage,
delay or injury to you, your travel companions
or group members. In addition, BlackNLA
cannot guarantee acceptable performance
by any of these firms and each participant
agrees not to hold BlackNLA liable for the
quality of services, the condition or cleanliness
of the accommodations, or any injuries sustained.
If the term "guaranteed" is specifically
stated in writing on your ticket, invoice
or reservation itinerary; the rate, booking,
service or reservation is guaranteed by
the stated supplier. Your retention of tickets,
reservations or bookings after issuance
shall constitute a consent of the above,
and an agreement on your part to convey
the contents hereto to your travel companions
or group members. BlackNLA herein gives
notice that we cannot be responsible for
any disruption of travel and/or unrest,
labor problems, mechanical or construction
difficulties, climatic aberrations, acts
of God, war, diseases, or any other circumstances
beyond its control.
All
final payments are due May 24, 2010. If
full payment has not been received for packages
your payment will be forfeited and your
space will become available for resale.
Refund/Cancellation Policy: ABSOLUTELY NO
REFUNDS! Packages are transferable. There
will be a fee of $50.00 to transfer package
to a different name.
On-site Registration Requires: Presentation
of the credit card used at the time of purchase
and valid ID. No wristbands will be issued
without credit card and proper identification.
Hotel Check-in Will Require: major credit
or debit card and valid ID for every room
This
agreement is in addition to the above and
should not be interpreted in any way to
limit the release heretofore agreed to between
myself and BLACKNLA.
